Terms and Conditions
- I agree to another round of 10 Exercise Coaching Sessions.
- I agree to the price of $450 for these sessions.
- I agree to complete all 10 sessions within 60 days or less of completing this form.
- I agree that any session cancelled by me on the day of a scheduled session will result in the loss of that session.
- I acknowledge that this is not a doctor-prescribed program and that I have been cleared by my physician to participate.
- I hereby release Wright Fitness & Nutrition from liability for accidental injury, illness, or damage to my health that I may incur as a result of participating in this program.
- I hereby assume all risks connected therewith and consent to participate.
